Himachal Pradesh Council Shimla, has started facility for the medical practitioners is apply for Provisional as well as Permanent Registration, before applying for any registration one can seek help for uploading, filing required documents. To get properly registred:

Applicant has to

  • Fill all fields properly, Viz. Name, Reg. No., Phone/ Contanct No, email etc. correctly as these fileds will appear on certificates to be issued by the council.

  • Photograph to be uploaded should be clear, colored.

  • Make sure all documents are realdable.

  • Applicant signature should be clear and of appropriate size , as they will also be printed on certificate and application forms.

  • All documents should be properly scanned with size not more than 200kb for convenience applicants can use Image Cropper which can compress , resize the document.

  • All documents should be in jpg/png format.

  • Aministration can serve better of all documents are correct aunthentic and properly delivered/ uploads for future certification.

Autofill - Facility in Filling Forms :

Applicant can use the autofill facilities to reduce the typing again and again the fileds which is once typed if reference details are required one has to outer the registration number of person in refernce

Automatic filled some fields, like referance of other doctor if needed while applicant is applying the new application, applicant has to only enter the registration number of the doctors and the system will automatically filled the other fields related to that doctor.

Payment

Applicant can pay the fees only after successfully submitting / uploading required documents and will get the receipt after payment is successfully debited in HPMC A/c.

After submitting all documents and payment authorities will convey you the next process by the portal/ email / telephonically.

Provisional Registration

Any person, who has passed the qualifying examination of any University or Medical Institution in India for the grant of a recognized medical qualitication shall be entitled to be registered provisionally for the purpose of internship training and on an application made for provisional registration to the Registrar, the Registrar will issue a certificate of provisional registration.

To apply Provisional Application, applicant required following scanned certificate in jpeg/jpg/png/gif format and the size of the certificate should not be more than 200kb.

Documents required to apply:

Indian Graduates :

  • MBBS 1st year certificate.
  • MBBS 2nd year certificate.
  • MBBS 3rd year certificate.
  • MBBS 4th year certificate.
  • Attempt certificate. (if any)
  • Domicile certificate /Aadhaar card. (if graduate from outside of himachal)
  • Signature Specimen form. Download

Foreign Graduates :

  • Eligibility Certificate.
  • MCI Screening Test Report.
  • Character/Migration/NOC/No Criminal Certificate. (any supporting document)
  • Internship Permission Letter from College.
  • Qualification 1st year certificate.
  • Qualification 2nd year certificate.
  • Qualification 3rd year certificate.
  • Qualification 4th year certificate.
  • Attempt certificate. (if any)
  • Domicile certificate /Aadhaar card. (if graduate from outside of himachal)

Permanent Registration

Duplicate Certificate (Provisional / Permanent)

Issue of duplicate registration certificate-If a registration certificate is lost, Destroyed or mutilated, the registered practitioner may at any time during which the certificate is in force, apply to the Registrar for a duplicate certificate and the Registrar shall on being satisfied issue, on receipt of fee prescribed, on funishing an indemnity bond and after due confirmation and approval of the Executive Committee, a duplicate certificate. The duplicate certificate shall be inscribed on the top of the certificate.

To apply Duplicate Certificate for Provisional or Permanent, applicant required following scanned certificate in jpeg/jpg/png/gif format and the size of the certificate should not be more than 200kb.

Application Form Registration For Additional Qualification

Entries in the Register regarding additional qualifications (1) Persons registered with the Council are entitled to apply for entering additional qualifications acquired by him as specified in the Schedule to the Indian Medical Council Act, 1956 (Central Act No.102 of 1956) in Form on the Council’s register on payment of the prescribed fee.

To apply Additional Qualification, applicant required following scanned certificate in jpeg/jpg/png/gif format and the size of the certificate should not be more than 200kb.

  • Copy of Additional Qualification Certificate Attested.
  • Permanent Registration Cerificate.
  • Specimen Signature Attested Form.
Renewal

(1) The registration certificate can be renewed by the Council on receipt of application in renewal Form along with the fee prescribed for the renewal of the registration.

(2) The Registrar shall consider the application made under sub-rule (1) and if found fit shall renew the registration.

CME

Before apply Renewal kindly read instruction of CME.

CME records shold be updated as per rule and instruction of CME. Applicant has to update the CME record time to time. Portal has facility to update CME records online without any fee and then applicant can apply for the Renewal.

To apply Renewal application, applicant required following scanned certificate in jpeg/jpg/png/gif format and the size of the certificate should not be more than 200kb.

  • Certificate (Last Registered/Renewal).
  • Character Certificate.
NOC

NOC is leaving process form the council. If renewal is not pending,practitioner can get the NOC form the depatrment any time. After clearing all dues practitioner can apply for the NOC, if renewal is pending with the council so applicant can not apply for the NOC. NOC will issued only for the specific State of the India.

To apply NOC , applicant required following scanned certificate in jpeg/jpg/png/gif format and the size of the certificate should not be more than 200kb.

  • Certificate (Last Permanent Registered/Renewal)
Migration from Provisional to Permanent

This is a process of registration from the provisional to permanent. Applicant, who has completed his/her internship can migrate from provisional registration to permanent registration.

To apply Migration form Provisional to Permanent, applicant has to keep following scanned certificate in image(jpeg/jpg/png/gif) format and the size of the certificate should not be more than 200kb.

  • Copy of Additional Qualification Certificate Attested.
  • Permanent Registration Cerificate.
  • Specimen Signature Attested Form.
Re-Registration

After NOC has taken, if practitioner would like to register again with the council then login into your account and choose registration option apear on the dashboard. Through this option practitioner can active their account with council.

To apply Re-Registration, applicant required following scanned certificate in jpeg/jpg/png/gif format and the size of the certificate should not be more than 200kb.
